Registration Form
The priority registration deadline is September 24, 2004. The registration forms are available in a pdf format that may be edited directly. The conference fee applies to all participants , but will be refunded to those presenting papers at the conference. Participants with an academic affiliation should use the academic form. All others should use the standard form. The completed form should be sent:
- by e-mail attachment to econ-conference@uwo.ca if you have Adobe Acrobat 5.0 or higher. If you do not have Adobe Acrobat, use Adobe Reader to edit and print the completed form and fax or mail it.
- by fax to 519 661 3666 - attention Paula Nopper
- by regular mail to Paula Nopper, Department of Economics, Faculty of Social Science, The University of Western Ontario, London, Ontario, Canada, N6A 5C2
